DEFINITIONS:

5 Year Consolidated Plan (ConPlan):  The Consolidated Plan is prepared every 5 years and is designed to help states and local jurisdictions to assess their affordable housing and community development needs and market conditions, and to make data-driven, placed-based investment decisions. The consolidated planning process serves as the framework for a community-wide dialogue to identify housing and community development priorities that align and focus funding for the CPD formula block grant programs. 

Annual Action Plan (AP): The Consolidated Plan is carried out through Annual Action Plans, which provide a concise summary of the actions, activities, and specific federal and non-federal resources that will be used each year to address the priority needs and specific goals identified by the Consolidated Plan. 

Consolidated Annual Performance and Evaluation Report (CAPER):  Report used by Grantees to report on accomplishments and progress toward Consolidated Plan goals.

Substantial Amendment:  In general an amendment to an approval ConPlan or AP. A substantial amendment is required under any one of the following circumstances:
  • The grantee changes its allocation priorities or the method of funds distribution. 
  • The grantee wishes to carry out an activity that was not previously described in the action plan substantial amendment. 
  • Thee grantee changes the purpose, scope, location or beneficiaries of an activity.
